Requirements

A Master’s degree from an accredited Nurse Practitioner program
An active (or eligible) Indiana RN license in good standing
Nurse Practitioner certification (Family Practice, Psych/Behavioral Health, or Pediatrics)
Eligibility or active prescriptive authority/DEA licensure
A valid driver’s license, reliable vehicle, and current auto insurance

Nice To Haves

Preferably 2+ years of related experience
Community health or mental health experience

Responsibilities

Deliver comprehensive nursing care using advanced clinical knowledge and evidence-based practice
Conduct physical exams across diverse populations using a structured, developmental approach
Make differential diagnoses and identify both medical and non-medical contributors to health
Create individualized treatment plans that improve outcomes and support long-term wellness
Collaborate with patients and families to build meaningful, personalized health plans
Ensure accurate, timely documentation aligned with Meridian standards
Champion ethical, compliant care in accordance with all regulations
