Primary Care Nurse Practitioner/Physician Assistant - Part-Time
About The Position
Eventus WholeHealth is seeking a part-time Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to provide comprehensive primary care in skilled nursing and senior living communities. The role includes routine rounding, proactive chronic disease management, coordination of care across interdisciplinary teams, and collaboration with facility partners. The organization’s value-based approach emphasizes preventive care, improved quality outcomes, and a better experience for patients, families, and care teams. Clinicians use AI-enabled tools to support clinical decision-making, streamline workflows, and provide proactive, whole-person care. The schedule has no call and no weekends.
Requirements
- Current Nurse Practitioner or Physician Assistant licensure in the state of practice.
- Previous professional patient care experience.
- Exceptional patient-centered service skills, including compassionate communication, active listening, and professionalism.
- Ability to build trusting relationships with residents, families, and facility staff.
- Strong collaboration skills and ability to work effectively across interdisciplinary teams.
