About The Position

Revere Medical is seeking an experienced Primary Care Advanced Practitioner to join a well-respected and very busy community-based practice in Salem, New Hampshire. Ideal candidates will possess excellent clinical, collaborative, and communication skills, be comfortable using modern healthcare technologies and electronic medical records and have a strong commitment to serving our patient population. This is a wonderful opportunity to become part of a well-established primary care team, joining our existing physician and physician assistant. Requirements

Graduate of an accredited school of nursing
Graduate of an accredited Nurse Practitioner or Physician Assistant program
Current Nurse Practitioner or Physician Assistant certification
Comfortable using new technology and software
Minimum of one year of APP experience
Ability to work both independently and collaboratively within a team environment

Responsibilities

Provide comprehensive care to a diverse patient population, including children, adults, and geriatric patients
Conduct thorough physical exams, diagnose medical conditions, and develop personalized treatment plans
Promote health education and disease prevention
Order and interpret diagnostic tests and manage chronic conditions
Collaborate with specialists and other healthcare providers to ensure coordinated care
Maintain accurate up-to-date medical records and patient charts
Stay current with medical advancements and best practices in Primary Care
