About The Position
Center of Hope, part of Saint Francis, is seeking a Primary Care- Advanced Practice Provider to support continued primary care expansion and access initiatives across the Delaware market. This is a full-service Family Medicine Center providing primary care to a medically underserved population in the Newark area, with a strong focus on the Latino community. The practice emphasizes cultural component care, supported by a bilingual care team committed to delivering inclusive, patient-centered services.
Requirements
- PA or NP from an accredited institution
- Eligible for Delaware licensure
- Professional working proficiency in Spanish
- Commitment to high-quality, patient-centered care

Responsibilities
- Provide comprehensive primary care for patients across the lifespan
- Assist with the diagnosis and management of acute and chronic conditions

- Maintain timely and accurate documentation
- Participate in quality and performance improvement initiatives
