This is a full-time opening for a Primary Care Clinic in Perryman, with Monday-Friday, 4x10 shifts a week. We are looking for an experienced APP but are open to New Grads as well. The APP will be accountable for advanced nursing practice in the care of assigned patient populations. This role involves collecting and analyzing client health data, identifying appropriate expected outcomes, developing and implementing a plan of care, providing comprehensive clinical coordination of care and case management, and consulting to influence the plan of care for clients using prescriptive authority procedures and treatment in accordance with state and federal laws and regulations. The APP will also evaluate clients' progress in attaining expected outcomes, conduct nursing research, and provide education to patients, families, and nursing staff. The role includes delivering comprehensive care to new and established patients, participating in continuing education, serving as a consultant and leader, participating in the call schedule, assuring quality standards and excellent patient experience, meeting RVU productivity goals, participating in provider meetings, completing patient paperwork and responding to Inbasket messages, performing documented procedures consistent with training and experience, developing high-quality visit notes, utilizing critical thinking for physician collaboration, maintaining DEA and BNDD requirements if applicable, ordering and interpreting diagnostic studies, and providing personalized patient education.
