About The Position

We’re seeking high-quality and compassionate Advanced Practice Provider (NPs or PAs) to join our dynamic primary care team. Responsibilities

Provide comprehensive, longitudinal patient-centered care in an outpatient setting while collaborating with an experienced multidisciplinary team.
Perform comprehensive patient evaluations and physical examinations
Diagnose and manage common acute illness and chronic conditions, including hypertension, diabetes, asthma, and other common primary care conditions
Develop, implement and evaluate individualized care plans
Order, interpret, and follow up on labs, imaging, and other diagnostic tests
Prescribe medications and treatments consistent with current clinical guidelines
Provide age-appropriate care, including adult and geriatric care (and pediatrics, if capable)
Coordinate in transitions of care, including hospital follow-ups and post discharge management
Collaborate with Bayview specialists through an integrated internal referral network
Work and collaborate with other Physicians and APPs within the practice
