About The Position

Bayview Physicians Group, a large independent, multi-specialty, physician-led group in Virginia, is seeking compassionate Advanced Practice Providers (NPs or PAs) to join their primary care team in Southeast Virginia, including Virginia Beach, Chesapeake, Norfolk, and Suffolk. The role involves providing comprehensive, longitudinal, patient-centered care in an outpatient setting, collaborating with an experienced multidisciplinary team, and managing a patient panel of approximately 18-20 patients per day. The group emphasizes a strong commitment to patient-centric, value-based care within an established, integrated healthcare network.

Requirements

  • Ability to provide comprehensive, longitudinal patient-centered care in an outpatient setting
  • Ability to collaborate with an experienced multidisciplinary team
  • Ability to diagnose and manage common acute illness and chronic conditions
  • Ability to develop, implement and evaluate individualized care plans
  • Ability to order, interpret, and follow up on labs, imaging, and other diagnostic tests
  • Ability to prescribe medications and treatments consistent with current clinical guidelines
  • Ability to provide age-appropriate care, including adult and geriatric care (and pediatrics, if capable)
  • Ability to coordinate in transitions of care, including hospital follow-ups and post discharge management
  • Ability to collaborate with Bayview specialists through an integrated internal referral network
  • Ability to manage approximately 18-20 patients per day
  • Ability to work and collaborate with other Physicians and APPs within the practice

  • Experience with eClinicalWorks (eCW) EMR platform

Responsibilities

  • Perform comprehensive patient evaluations and physical examinations
  • Diagnose and manage common acute illness and chronic conditions, including hypertension, diabetes, asthma, and other common primary care conditions
  • Develop, implement and evaluate individualized care plans
  • Order, interpret, and follow up on labs, imaging, and other diagnostic tests
  • Prescribe medications and treatments consistent with current clinical guidelines
  • Provide age-appropriate care, including adult and geriatric care (and pediatrics, if capable)
  • Coordinate in transitions of care, including hospital follow-ups and post discharge management
