About The Position

The PrEP Navigator plays a crucial role in supporting individuals at risk of HIV infection by providing education, guidance, and assistance in accessing pre-exposure prophylaxis (PrEP), post-exposure prophylaxis (PEP) and DoxyPEP services. The PrEP Navigator will work closely with both internal and external healthcare providers, community organizations, and individuals to promote awareness and facilitate the use of preventive measures against HIV and STI’s.

Requirements

  • Knowledge of HIV prevention strategies, PrEP, PEP and DoxyPEP.
  • Strong communication and interpersonal skills with the ability to deliver critical program information at the clients’ level.
  • Ability to work collaboratively with diverse populations.
  • Analytical skills to assess and resolve complex socio-economic issues effectively.
  • Proficiency in Microsoft Office and data management software.
  • Bachelor’s degree in Public Health, Social Work, or a related field (or related experience)
  • 1+ years of experience in HIV and/or STI prevention.
  • Experience in community outreach and education.
  • Strong understanding of HIV prevention strategies, PrEP, PEP and DoxyPEP.
  • Must maintain a valid driver’s license and current automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company’s auto insurance carrier if you drive company owned vehicle or your personal vehicle during company business.

Responsibilities

  • Provide clear and comprehensive information to individuals about HIV transmission, risk factors, and the benefits of PrEP/PEP
  • Conduct one-on-one, group or community sessions to educate clients on the efficacy, proper use and adherence to PrEP medication
  • Conduct initial assessments to determine individuals' eligibility for PrEP or PEP based on risk factors and medical history
  • Collaborate with healthcare professionals to ensure accurate eligibility determinations and linkage
  • Facilitate referrals to healthcare providers and clinics offering PrEP/PEP services
  • Coordinate appointments and follow-up care to ensure continuity of prevention services
  • Provide Transportation to and from medical appointments as needed
  • Provide administrative assistance for medication assistance programs or health insurance coverage as needed
  • Coordinate with pharmacies to ensure client receives or has access to medication
  • Engage in community outreach activities to raise awareness about PrEP/PEP and reduce stigma associated with HIV
  • Collaborate with community-based organizations to reach vulnerable populations
  • Maintain accurate records of client Intakes, interactions, assessments, medication assistance program enrollment, Insurance program eligibility and follow-up activities
  • Compile and analyze data related to PrEP/PEP utilization for reporting and program improvement
  • Enter Client information in DSHS Evalweb database
  • Provide support and guidance to individuals facing potential exposure to HIV, assisting them in accessing timely PEP services
  • Provide medication assistance for those needing PEP that do not have health insurance
  • Provide copay cards for clients needing support covering cost
  • Engage with healthcare providers, public health agencies, and community organizations to strengthen the continuum of care for ending the HIV epidemic
