About The Position

Under the direction of the Program Coordinator and Clinic Supervisor, the PrEP Navigator will have a crucial role in promoting awareness of HIV biomedical prevention services, specifically emphasizing pre-exposure prophylaxis ("PrEP") and post-exposure prophylaxis ("PEP"). The primary focus will be on populations that are particularly vulnerable to HIV. Their responsibilities encompass strengthening the capabilities of culturally-competent healthcare providers who prescribe PrEP/PEP, and serving as a direct link to care for individuals actively seeking access to these essential services.

Requirements

  • AA or BA degree in General studies/Public Health or a health/human service field or at least two (2) years equivalent experience in the HIV field.
  • Experience working with vulnerable populations.
  • Current HIV/AIDS, STI, and biomedical HIV prevention knowledge.
  • Effective oral and written communication skills.
  • Intermediate knowledge of word processing (MS Word) and spreadsheet (MS Excel) programs.
  • Current California Driver License and ability to drive to program sites.
  • One-year experience providing outreach, workshops or related activities.

Nice To Haves

  • Bilingual (English/Spanish) preferred
  • Experience and specific training/experience working with people living with HIV/AIDS preferred.

Responsibilities

  • Collaborate with clients at elevated risk of HIV to assess their eligibility for and facilitate their enrollment in PrEP and PEP medical services.
  • Provide support to the PrEP Program Coordinator in the implementation of County- and State-funded Biomedical prevention grants.
  • Assist in maintaining client records, updating information, and inputting patient-level data into systems such as Case-watch and/or LEO.
  • Conduct individual counseling and assessments for potential PrEP/PEP clients.
  • Engage in weekly collaboration with biomedical prevention staff to identify client needs and brainstorm innovative strategies to attract new patients to PrEP/PEP and other medical services.
  • Lead capacity-building activities related to PrEP/PEP within community-based organizations (CBOs) and other community health centers in LA County.
  • Deliver PrEP/PEP-related presentations and training sessions to staff, patients, and community members across diverse settings.
  • Attend professional conferences and summits to showcase JWCH/WHC's work and stay informed about the latest biomedical prevention trends and data.
  • Identify and actively participate in outreach events to recruit new PrEP/PEP and/or medical patients.
  • Represent JWCH/WHC in community planning groups, advisory boards, and committees.
  • Collaborate with other JWCH/WHC programs and satellite sites to ensure successful referrals for PrEP/PEP-related services and appointments.
  • Co-produce outreach and recruitment events with biomedical prevention staff to increase the visibility of the PrEP/PEP program.
  • Participate in both staff and department meetings.
  • Adhere to all organizational safety policies, procedures, and regulatory requirements to ensure a safe environment for patients, staff, and visitors.
  • Participate in required safety and compliance trainings and apply learned practices in daily work activities.
  • Identify, report, and help mitigate unsafe conditions, incidents, or hazards in the workplace.
  • Use equipment, tools, and supplies safely and responsibly in accordance with organizational and regulatory standards.
  • Maintain awareness of infection control, occupational health, and emergency preparedness procedures appropriate to the role.
  • Participate in JWCH’s customer service training called AIDET on an annual basis. Demonstrate an understanding of AIDET (Acknowledge, Introduce, Duration, Explanation, and Thank You) to apply in patient encounters or team discussions. Utilize the AIDET communication framework to communicate with patients and team members in a manner that reduces patient anxiety, increases patient compliance, and improves clinical outcomes.
