About The Position

The Telephonic Triage LPN participates in nursing care services for assigned practices and specialties. The role collects and documents objective and subjective patient observations by telephone and in the electronic medical record, using approved clinical protocols to guide patient direction and disposition. The LPN serves as a patient and caregiver advocate and as a communication link among patients or caregivers, physicians, pharmacies, and hospitals through approved communication platforms. The position requires recognizing emergent, urgent, and non-urgent needs and using clear, timely documentation, with the goal of resolving the first call when within scope. The role supports the mission and values of TriHealth Physician Practices. The LPN manages patient messages and concerns, conducts assessments and triage using clinical protocols and critical thinking, and communicates or hands off patient information to the healthcare team using SBAR. When protocols do not address a patient's needs, the LPN collaborates with an RN triage team member or communicates the information to a physician or APP. The position also supports patient education, quality and safety, positive patient experiences, collaborative team relationships, and culturally competent, age-appropriate care. The position is full-time, day shift, at 6010 S Mason Montgomery Rd., Cincinnati, Ohio, with a rotating weekend schedule.
