About The Position

The health services that became the Anthony L Jordan Health Corporation (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y. Jordan Health is now seeking a PPC RN who, under the guidance of the PPC Clinical Lead, is primarily responsible for assisting and supporting the PPC practice team by performing targeted task within RN scope as assigned by clinicians and/or Practice Manager. To ensure efficient and quality patient care, with a specialized focus on HIV/AIDS, Hepatitis, and PrEP/PEP management. Requirements

  • BSN/RN, minimum 2 years’ experience
  • Current and active NYS RN license without any negative actions
  • Current Phlebotomy certification (or demonstrated equivalent competency with documentation)
  • Demonstrated knowledge and clinical experience in HIV/AIDS treatment, Hepatitis, and PrEP/PEP, including medication regimens, adherence strategies, and risk-reduction counseling
  • Excellent computer skills, preferably with an electronic health record (eCW experience preferred)
  • Current provider BLS/CPR through the American Heart Association.
  • NYS Infection Control Certificate.
  • Strong writing and computer skills
  • Possess and utilize effective verbal and written communication
  • Ability to be self-motivated, work independently, and work as a team member
  • Ability to represent the agency in a professional manner within the community
  • Skills and competence to establish supportive, trusting relationships with people living with HIV/AIDS/Hepatitis and respect for client rights and personal preferences are essential
  • Proficiency in administering intramuscular, subcutaneous, and intradermal injections
  • Proficiency in venipuncture and capillary blood collection
  • Working knowledge of New York State DOH AIDS Institute guidelines, ADAP, and PrEP/PEP access pathways.
