Build your Career. Make a Difference. Presbyterian is hiring a skilled Population Health Digital Outreach Manager to join our team. This is a full-time, exempt position based at the Reverend Hugh Cooper Administrative Center with a weekday schedule, Monday-Friday.

Requirements

  • Bachelor’s degree or equivalent experience in healthcare, public health, or related field.
  • Experience in population health, healthcare operations, or digital engagement.
  • Hands-on experience executing outreach campaigns across digital platforms.
  • Knowledge of value-based care, STARS, HEDIS, and additional quality programs.
  • Strong collaboration and communication skills across multidisciplinary teams.

Nice To Haves

  • Experience with CRM platforms (e.g., Salesforce Health Cloud) preferred but not required.
  • Experience supporting implementation of new engagement tools.
  • Familiarity with Epic, Genesys, Jiva, or similar EHR/Telephony/Care Management systems.
  • Experience with Tableau, Power BI, or similar tools.

Responsibilities

  • Designing, executing, and optimizing digital engagement strategies to improve health outcomes and quality performance across Medicaid, Medicare, D-SNP, and Commercial populations.
  • Partnering across the health plan, care management, quality, and provider operations to ensure outreach is coordinated, data-driven, and aligned with clinical workflows and value-based care goals.
  • Supporting both current-state campaign execution and future-state capabilities, including population health use cases for future-state CRM implementation.
  • Driving staffing efficiencies by transitioning appropriate outreach from manual phone calls to digital channels (SMS, email, portal, IVR).
  • Increasing member participation in clinical, preventive, and care management programs through targeted digital engagement.
  • Supporting scalable outreach models that reduce operational burden while maintaining or improving outcomes.
  • Ensuring vendor-supported digital communications align with PHP strategy.
  • Owning end-to-end population health digital outreach campaigns across SMS, email, IVR, and patient portal channels.
  • Partnering with the Analytics Organization to translate targeting outputs into outreach execution.
  • Aligning outreach strategies with STARS, HEDIS, and state quality measures.
  • Coordinating with care management and provider teams to align outreach with clinical workflows.
  • Deploying campaigns across multiple tools while ensuring consistency and compliance.
  • Supporting implementation and operationalization of population health CRM capabilities.
  • Participating in requirements gathering, vendor evaluation, and user acceptance testing.
  • Monitoring campaign performance and driving continuous improvement.
  • Ensuring compliance with HIPAA, CMS, and communication regulations.
