About The Position

The Population Health Specialist will work closely with patients and others on the Care Team and within the Quality department to identify gaps in patient care, strategize workflows for gap closures and track clinical data. The Population Health Specialist will work in the electronic health record to identify opportunities to provide “best practice” services to patients with specified diseases or care needs. This position will work to improve overall quality and completeness of the medical record and outreach to patients as necessary.

Nice To Haves

  • Experience with Epic EHR or other electronic health record
  • Knowledge of quality measures including: QIM, HEDIS and UDS.
  • Experience scheduling patients
  • Experience in panel management
  • Current Oregon Driver’s License

Responsibilities

  • Review reports and clinical records to identify patients who need services defined by Primary Care and Quality leadership for preventive services, diagnostics, and follow up.
  • Coordinate services for targeted patients.
  • Ability to document progress and share with Quality Department and others
  • Outreach patients directly to discuss preventative care needs and arrange follow-up with providers. Utilize additional patient engagement tools such as letters, MyChart messages and other communication tools
  • Appropriate utilization of assistive devices and health care interpretive services as needed by those patients.
  • Update the medical record to reflect completion of recommended health maintenance and procedures.
  • Partner with medical records to obtain chart notes and results needed to close quality gaps.
  • Collaborate and coordinate with provider teams, front office, billing, quality, and others.
  • Participate and support evaluation of team data for proactive panel management.
  • Engage with provider teams and quality coordinator or others, on chart deficiencies requiring clarification.
  • Engage patients in scheduling follow-up appointments-with PCP for hospital discharges.
  • Perform other duties as assigned.
