About The Position

NeighborHealth is seeking an experienced Director of Population Health to support the strategic direction for population health across the organization. This role requires a deep understanding of and experience in population health principles, strategic planning, value based care, and change management. The Director will support the design and implementation of the long-term roadmap that advances value-based care, strengthens care delivery models, and improves outcomes for defined patient populations. The director works closely with executive leadership, clinical and operational leaders, and IT teams to build the systems, partnerships, and capabilities to implement population-level interventions and programs.

  • Experience supporting contract performance for ACOs, PCMH, MSSP, and other risk-bearing arrangements
  • Ability to ensure compliance with program requirements and timely submission of contract deliverables
  • Must complete a background check before beginning employment

Responsibilities

  • Develop and execute a population health strategy aligned with community needs, organizational goals, value-based care best practices, and regulatory requirements.
  • Lead initiatives that improve chronic disease management, preventive care, care transitions, and patient engagement.
  • Promote health equity through targeted interventions for high risk or underserved populations.
  • Identify emerging trends, gaps, and opportunities to enhance population health performance.
  • Work with clinical leadership to embed population health principles into primary care workflows and care pathways.
  • Lead cross-departmental initiatives to improve chronic disease management, preventive care, care coordination, and addressing patients’ health-related social needs at the population-level.
  • Partner with IT and analytics teams to build and expand data collection and reporting systems that can be used to understand trends in population characteristics, healthcare utilization, and performance on key performance indicators.
  • Translate data insights into actionable plans for clinical and operational teams.
  • Monitor key performance indicators across value-based contracts and internal quality programs.
  • Manage relationships with payer partners and support contract performance for ACOs, PCMH, MSSP, and other risk-bearing arrangements.
  • Ensure compliance with program requirements and timely submission of contract deliverables.
