About The Position

As a population health coordinator, you’ll have an important role in preventive care and in addressing social drivers of health, and will work with patients toward improving health outcomes. You will engage patients through motivational interviewing, use data to support care for high-risk populations, prepare clinical data for review, prepare workflows to support team-based care, and evaluate progress toward goals. Key responsibilities include coordinating high-risk patient management and outcomes, preparing and monitoring population health data, and coordinating communication and care across teams. You will be primarily based at our Riverway location, 1569 Niagara Street, Buffalo.

Requirements

  • Bachelor’s degree or equivalent professional experience
  • Experience in community health, patient resource coordination or equivalent
  • Computer skills, clerical skills, data entry and organization
  • Excellent written and verbal communication, and problem solving skills
  • Able to read, write and speak the English language.
