This position supports value-based care and quality improvement initiatives by identifying and helping resolve gaps in patient care. The role involves reviewing population health data, electronic health records, and clinical documentation to verify completion of preventive services and other quality measures, identify missing information or outstanding care needs, and support accurate quality reporting. The Population Health Coordinator works collaboratively with physicians, clinical staff, and other departments to improve quality outcomes by researching patient records, maintaining accurate documentation, coordinating follow-up on identified care gaps, and supporting workflows designed to improve preventive and chronic care.
