The Population Health Coordinator supports value-based care and quality improvement initiatives by identifying and helping resolve gaps in patient care. This position reviews population health data, electronic health records, and clinical documentation to verify completion of preventive services and other quality measures, identify missing information or outstanding care needs, and support accurate quality reporting. Working collaboratively with physicians, clinical staff, and other departments, the Population Health Coordinator helps improve quality outcomes by researching patient records, maintaining accurate documentation, coordinating follow-up on identified care gaps, and supporting workflows designed to improve preventive and chronic care.
