About The Position

The Population Health Associate functions to support population management activities. The Population Health Associate collaborates with Nurse Care Managers and the entire care team to help coordinate population health-related services with the goal of achieving positive health outcomes. Job responsibilities include, but not limited to, using reports to identify patients who need periodic follow-up gaps in care, lapses in care, and condition specific testing/screening. These interactions will primarily be achieved through outreach via telephone, e-mail, or mail.

Requirements

High School diploma or GED
A minimum of one (1) year of clinical experience as a Licensed Practical Nurse or Medical Assistant in clinic setting
Previous experience with Electronic Medical Records (EMR)
Previous experience with Microsoft Office (Word, Excel, Outlook)

Nice To Haves

Licensed as a Licensed Practical Nurse in the state of Missouri
Certification as Medical Assistant required by a nationally recognized credentialing organization
