About The Position

Pinnacle Home Care, Florida’s largest independent Medicare-certified home health provider, is seeking a Platform Coordinator to join its award-winning team. This role is crucial in supporting Care Coordinators by managing platform referrals. The Platform Coordinator will be responsible for processing patient referrals from various platforms, reviewing documents, confirming, and processing them through Pinnacle’s EMR. The position requires a high level of patient focus, involving direct interaction with Care Coordinators, Case Managers, Physicians/PCP(s), insurance representatives, outside vendors, and other medical personnel. The ideal candidate will be detail-oriented, capable of multitasking, and open to cross-functional training across departments. This role is integral to delivering high-quality, patient-centered care at home.

Requirements

  • Must have a GED or High School Diploma.
  • Ability to manage conflict, stress, and multiple simultaneous work demands in an effective, professional, positive manner.
  • Analyzing and reconciling complex issues and problems timely.

  • Follows directions and respectfully requests clarification when needed.

  • Working knowledge of EMR database: Allscripts, Navi, Epic NCH, PowerChart.
  • Proficient with Microsoft Word, Excel, and Outlook software.
  • Possesses good customer relations skills and a commitment to providing quality service.
  • Must possess excellent prioritization and time management skills.
  • Must be able to communicate effectively and clearly.

  • Associate's, or Bachelor’s Degree in a related field.

Responsibilities

  • Effectively prioritize and complete tasks to support Care Coordinators with platform referrals.
  • Coordinate to process patient referrals from Navi, Allscripts, Epic NCH, and Power chart, as well as other referral platforms.
  • Pull referrals off platforms to accept/decline.
  • Review documents, confirm, and process the referral through Intake using Pinnacle’s EMR.
  • Respond to Care Coordinator inquiries regarding referral status promptly.
  • Respond to all inquiries with a resolution to concerns and or discrepancies.
  • Coordinate and communicate with the appropriate team regarding any changes or updates to the patient’s payer source on time.
  • Meet productivity goals as assigned by the direct supervisor.
  • Comply with HIPAA regulations on all accounts.
  • All other responsibilities assigned by leadership.
