About The Position

The Placement Liaison acts as the primary point of contact between the DHS Central Referral Unit (CRU), APM Case Management, and Placement Provider agencies. Asociación Puertorriqueños en Marcha (APM) has been helping families achieve their greatest potential since 1970 by providing early childhood education, foster care & adoption, child welfare, mental & behavioral health, community and economic development, housing, community school services, and violence intervention services to the North Philadelphia communities. The Community Umbrella Agencies (CUA) are community-based agencies established to provide case management services and ensure the permanency, wellbeing, and safety of the children, youth, and their families. APM CUA is looking for compassionate and committed social workers who want to create long-lasting impacts in the lives of children and families. APM CUA 2 services the Lower Eastern North region and CUA 5 services the Logan/Olney region.

Requirements

  • High School Diploma or equivalent is acceptable, however a bachelor's degree is preferred
  • Proficiency in English and Spanish preferred.
  • Strong organizational skills.
  • Demonstrated written and oral communication skills
  • Demonstrated ability to work independently and as a team.
  • Ability to set priorities in critical situations.
  • Strong Customer Service insight.
  • An applied understanding of general ethics and confidentiality.
  • Maintain a high degree of discretion dealing with confidential information.
  • Basic proficiency in Microsoft Office Applications.

Responsibilities

  • Acts as a liaison between CRU, Case Management, and Placement Providers.
  • Notification to CRU of respites, discharges, changes of services.
  • Provides notification of other changes of services, e.g., emergency kinship placements, kinship certification, Runaways, hospitalizations, and RTF placements.
  • Update service codes in the Philadelphia Family Data System (PFDS)
  • Open and close services in PFDS
  • Will function as APM CUA’s SWAN liaison; responsible for tracking SWAN referrals and Payment Authorization Forms
  • Responsible for submitting all Request for Court Ordered Retro Payments and CUA Direct Payment Forms (if applicable).
  • May be on call as needed.
  • May be responsible for other duties as assigned by Intake Manager as needed.
