About The Position

CHD is seeking an Outreach Caseworker to join our growing Planning and Implementing Housing Options for Long-term Success (PILOTS) team in Danbury, CT. Through this program, the Outreach Caseworker PILOTS staff will use a counseling approach that uses both motivational interviewing and harm-reduction. As a PILOTS Support Specialist, you will assist program participants to live independently in the community by addressing their mental and physical health, substance use, employment and housing needs. PILOT’s staff use a counseling approach that uses both motivational interviewing and harm-reduction.

Requirements

High school diploma or GED required
Experience working with psychiatric disorders or co-occurring population preferred
Must be comfortable engaging clients in the community for majority of the position
Field visits within the surrounding communities to see clients
Registered & insured vehicle and approved driving record check
Approved criminal background check

Responsibilities

Securing housing, move-in, relocations, interventions in eviction processes, rent arrears and housing stability.
Assist participants with budgeting/banking
Help participants with obtaining and maintaining entitlements.
Provide crisis intervention
Provide vocations assistance; resume writing, interview preparations, and job search assistance.
