About The Position

Health Sciences North (HSN) is seeking an innovative individual for the position of Physiotherapist to use a patient centered approach to collaborate within the multi-disciplinary team to provide effective assessments and treatments to support the patient’s ongoing management of chronic pain. The clinic is open to patients over the age of 18 through referrals by a primary care provider (PCP). The Integrated Chronic Pain program accepts referrals for patients living in Northeastern Ontario. We are a Ministry of Health and Long-Term Care (MOHLTC) OHIP-referral funded program. In our model of care, PCPs play an active role in the treatment of their patients. We provide education-based programing to patients in order to support self-management of their chronic pain condition. This program integrates an inter-professional team (Registered Nurse, Occupational Therapist, Pharmacist, Physiotherapist, Recreational Therapist, Kinesiologist, Case Manager and Social Worker), with a focus on rehabilitation, self-management and setting functional goals in a group environment. Patients are expected to have an active role in their care.

Requirements

  • Current Certificate of Registration in good standing with the College of Physiotherapists of Ontario.
  • Current certification in Basic Cardiac Life Support (BCLS) and Automated External Defibrillator (AED) is required.
  • Certificate in Non-violent Crisis Intervention course is required.
  • Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
  • A minimum of three (3) years’ independent practice as a physiotherapist.
  • Demonstrated ability to direct, supervise, and mentor students and support personnel.
  • Demonstrated current knowledge of trends in general health care and community resources.
  • Demonstrated knowledge of Indigenous and Francophone culture.
  • Demonstrated knowledge and use of the Ontario Telemedicine Network (OTN; OTN-PCVC) and willingness to use in clinical practice.
  • Demonstrated knowledge of Medical Terminology.
  • Demonstrated excellent judgement and proven analytical skills.
  • Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
  • Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  • Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
  • Demonstrated superior interpersonal and communication skills, both written and verbal.
  • Demonstrated strong organizational skills with the ability to prioritize workload.
  • Demonstrated discretion and maturity when handling confidential information.
  • Demonstrated commitment to the safety of co-workers and patients.
  • Successful Vulnerable Sector Check is required.
  • Ability to use tact and discretion in dealing with health care providers and patients.
  • Proven ability to work independently and in a team environment.

Nice To Haves

  • Certification as an Assistive Devices Program (ADP) authorizer is preferred.
  • Experience in chronic pain is preferred.
  • Experience working in a functional pain program including the utilization of physical and psychosocial interventions is preferred.
  • Experience working with people who have severe and persistent physical and/or mental illness is preferred.
  • Experience working in an inter-professional team is preferred.
  • Experience working in assessment and treatment of out-patient musculoskeletal conditions is preferred.
  • Valid driver’s license with access to reliable vehicle may be required.
  • Ability to travel between local sites and willingness to travel occasionally within Northern Ontario.

Responsibilities

  • Perform detailed, comprehensive medical history and physical assessment of patients to establish a clinical impression for physiotherapy intervention as required.
  • Initiate and execute all physiotherapy treatments related to the clinical impression to deliver patient care, and perform on-going evaluations to determine when discharge from service is appropriate.
  • Rehabilitate injuries and the effects of disease/disability by implementing therapeutic exercise programs and other interventions.
  • Educate clients and families in coping strategies, decision-making, and utilization of personal and community resources, and make referrals to community resources as required.
  • Assist with program development activities, including liaising with inter-professional team members and partner sites as required.
  • Document assessment in the patient’s medical chart, clinical impression, and all related physiotherapy intervention and patient outcomes; maintain an accepted standard of documentation for quality assurance as outlined by HSN policies and the College of Physiotherapists of Ontario (CPO).
  • Communicate verbally or through written reports with other health care professionals regarding patient management for clinical problem-solving and program development.
  • Act as a professional resource for internal health care providers and community partners.
  • Provide theoretical and clinical education/supervision to physiotherapy students and undergraduate students.
  • Provide possible home visits as required.
  • Develop and implement self-learning objectives, participate in related postgraduate educational opportunities to improve clinical expertise, and integrate new knowledge to enhance patient care.
  • Participate on patient care teams and other committees, including Professional Council meetings, regarding the development of policies and procedures, forms and audits, and coverage/vacation planning within the professional discipline.
  • Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
  • Actively participate in program research activities as required.
  • Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
  • Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  • Contribute to learner development by providing academic support, mentorship, preceptorship and supervision as required.
  • Educate and promote health, safety and wellness in the work place.
  • Represent the department or program on various committees and in meetings as required.
  • Perform other duties as required.
