Physician

InnovAge•Orlando, FL
•Onsite

About The Position

The Physician provides medical care to InnovAge’s enrolled geriatric participants in a variety of settings, including the Day Center clinic, participants’ homes, nursing homes, assisted living facilities, and personal care boarding homes. Most visits take place at the InnovAge Center clinic, though some physicians may specialize in nursing home care. The role requires close coordination with hospital-based providers and physical attendance at an InnovAge facility. The schedule is Monday through Friday, 8 a.m. to 5 p.m., with four days of participant visits and one administrative day, and no regular nights, weekends, or holidays. The Physician evaluates, diagnoses, and manages participants’ medical conditions; documents care in accordance with InnovAge PACE policies; participates in interdisciplinary care planning and quality improvement; and helps develop medical programs and processes. Physicians also provide rotational after-hours phone coverage every few weeks. Calls are triaged by an RN, and only a subset reach the Physician for advice or orders; this coverage does not require travel to a center or home. InnovAge’s PACE program provides customized healthcare and social support to help seniors live independently in their homes and communities. Physicians work with colleagues in a patient-centered care model to create personalized care plans.
