About The Position

ONE Health Ohio, a Federally Qualified Health Center (FQHC), is seeking a dedicated Family Practice Physician to provide high-quality, compassionate care to patients of all ages. The role calls for a strong clinician and communicator who is passionate about collaborative, team-based care. The physician may work at any ONE Health Ohio location as needed and may perform other related duties to meet the organization’s ongoing needs.

Requirements

• Board certified or board eligible.
• Graduated from an accredited school of medicine.
• Licensed to practice medicine in Ohio and in good standing.
• Current DEA certificate.
• Completion of an accredited residency program.
• Valid Ohio driver’s license.
• Current BLS certification.

Responsibilities

• Provide comprehensive medical care, including diagnosis and treatment of acute and chronic illnesses and conditions.
• Educate patients and families about diagnoses, treatment plans, preventive care, and safety while following HIPAA and other regulatory guidelines.
• Collaborate with nurse practitioners, physician assistants, and other healthcare providers.
• Collaborate with, supervise, and mentor mid-level providers and students as needed.
• Support multidisciplinary care with clinicians, behavioral health providers, and other team members.
• Provide patient education on health promotion, disease prevention, and condition management.
• Provide ongoing follow-up, continuity of care, and chronic disease management.
• Maintain accurate and timely documentation in the electronic health record system.
• Uphold ONE Health Ohio’s core values and policies.
• Communicate openly with supervisors and colleagues.
• Maintain a positive and respectful attitude in all interactions.
• Remain flexible and adapt to changing tasks.
