We are seeking a skilled and compassionate Physician to join our Virginia Beach team on a part-time basis. This role offers the opportunity to provide high-quality medical care within post-acute, long-term care and assisted living settings. The successful clinician will work collaboratively with interdisciplinary teams, support patients and families, and promote continuity of care in a supportive clinical environment.

Requirements

  • MD or DO degree from an accredited medical school
  • Current, unrestricted Virginia medical license and eligibility to practice in Virginia
  • Current DEA registration and prescriptive authority, as required
  • Board certification or board eligibility in Family Medicine, Internal Medicine or a related medical specialty
  • Proficiency with Electronic Medical Record systems and timely, accurate documentation practices
  • Excellent communication, collaboration and problem-solving abilities
  • Compassionate, culturally sensitive and person-centered approach to patient care
  • Ability to work independently while collaborating effectively with facility staff and multidisciplinary care teams

Nice To Haves

  • Experience in post-acute, long-term care, geriatric, primary care or a related clinical setting preferred
  • Strong clinical assessment, diagnostic reasoning and patient management skills

Responsibilities

  • Provide comprehensive medical care to patients in post-acute, long-term care and assisted living settings
  • Conduct initial and follow-up assessments, including reviews of medical history, physical examinations and clinical status
  • Diagnose and manage acute and chronic medical conditions in accordance with current clinical standards
  • Develop, implement and regularly review individualized treatment plans in collaboration with patients, families, facility staff and interdisciplinary teams
  • Prescribe, adjust and monitor medications safely and effectively, including consideration of potential interactions and adverse effects
  • Recognize and respond appropriately to changes in patient condition, including urgent and emergent medical needs
  • Coordinate transitions of care, hospital follow-up, specialist referrals and other healthcare services
  • Provide education and support to patients, families and facility staff regarding diagnoses, treatment plans, medications and preventative care
  • Participate in care planning meetings, interdisciplinary reviews and family consultations as required
  • Communicate effectively with nursing staff, specialists, hospitals, therapists and other members of the care team
  • Maintain accurate, timely and clinically appropriate documentation in the Electronic Medical Record (EMR)
  • Contribute to quality improvement initiatives and promote evidence-informed, culturally sensitive and person-centered care
