Summary We are seeking a compassionate and skilled physician to join our healthcare team, delivering high-quality medical care in an Inpatient Rehabilitation Facility (IRF). In this role, you will provide direct patient care through daily rounding, focusing on managing medical comorbidities, supporting functional recovery, and coordinating care with an interdisciplinary rehabilitation team. Requirements

  • MD/DO degree from an accredited medical school
  • Board Certified in Internal Medicine, Family Medicine, Geriatrics, or Hospital Medicine
  • Current unrestricted license to practice medicine in the state of employment
  • Active DEA License
  • Proficiency with EMR

Responsibilities

  • Perform comprehensive medical assessments, develop and manage treatment plans, and provide ongoing medical oversight for IRF patients throughout their rehabilitation stay, with a focus on managing comorbidities and supporting recovery.
  • Conduct structured daily rounds to assess patients' medical status, adjust treatment plans, manage medications and orders, and address any emergent health concerns that arise during the rehabilitation course.
  • Collaborate with the interdisciplinary team — including physical therapists, occupational therapists, speech-language pathologists, case managers, and nursing staff — to ensure comprehensive, coordinated care tailored to each patient's rehabilitation goals.
  • Partner with case management and the rehabilitation team to assess patients' medical readiness for discharge and support appropriate transitions to home, outpatient therapy, skilled nursing, or other levels of care.
  • Educate patients, families, and facility staff on medical conditions, medications, and care management strategies that support recovery and reduce the risk of complications or readmission.
  • Maintain accurate, timely, and thorough documentation within the electronic medical record (EMR), ensuring compliance with IRF regulatory standards, CMS requirements, and organizational protocols.
  • Evaluate and manage acute changes in patient condition, initiate appropriate interventions, and determine when specialist consultation or transfer to a higher level of care is warranted.
  • Participate in quality improvement initiatives to enhance care delivery, reduce preventable hospital transfers, and optimize patient outcomes and satisfaction within the facility.
