About The Position

Colorado Physician Partners, PLLC is looking for a Primary Care Physician to join our clinic in Colorado Springs, Colorado. This is one of 12 clinics in an independent private practice in the Colorado Springs area. Our practice is a member of the independent practice association (IPA), PHPprime, which provides wrap-around services to ensure practice success. These services include care coordination, practice transformation coaching, risk adjustment coding education, population health management, data and analytics, specialist network management, and large group contracting. Our clinics are independent, outpatient private practices within the Denver area. The team consists of Providers, support staff, a social worker, and an RN/patient educator who serve as care coordinators. Most of our practices also offer mental health support. This practice is an active participant in the largest IPA in Colorado, and physicians hold clinical faculty appointments with the University of Colorado. We are known as the "go-to" group in the area for complex patients requiring the best medical care. Our physicians' clinical excellence has earned maximum scores/bonuses with CPC+, and the practices anticipate continued high performance.
