UW Health in northern Illinois seeks a BE/BC Family Medicine Physician to join the more than 370 employed providers of the SwedishAmerican Medical Group in Rockford, Illinois. Requirements
- MD or DO – Required
- Completion of an accredited Family Medicine residency program in the United States - Required
- Active DEA license or ability to apply for such license prior to or upon hire - Required
- Active Illinois medical license - Required
- Board Certification or Board Eligibility in Family Medicine. Board certification must be obtained within 5 years - Required
