Requirements

  • MD or DO – Required
  • Completion of an accredited Family Medicine residency program in the United States - Required
  • Active DEA license or ability to apply for such license prior to or upon hire - Required
  • Active Illinois medical license - Required
  • Board Certification or Board Eligibility in Family Medicine. Board certification must be obtained within 5 years - Required

Nice To Haves

  • Academic and teaching appointments are available with the University of Illinois College of Medicine at Rockford.

Responsibilities

  • Provide comprehensive family medicine care.
  • Develop a practice that aligns with professional goals, clinical interests, and preferred practice style.
  • Work with medical students and family medicine residents (if academic and teaching appointments are pursued).
