Requirements
- MD or DO – Required
- Completion of an accredited Family Medicine residency program in the United States - Required
- Active DEA license or ability to apply for such license prior to or upon hire - Required
- Active Illinois medical license - Required
- Board Certification or Board Eligibility in Family Medicine. Board certification must be obtained within 5 years - Required

- Academic and teaching appointments are available with the University of Illinois College of Medicine at Rockford.
Responsibilities
- Provide comprehensive family medicine care.
- Develop a practice that aligns with professional goals, clinical interests, and preferred practice style.
- Work with medical students and family medicine residents (if academic and teaching appointments are pursued).
