About The Position

Sutter Medical Group is hiring primary care physicians for Sutter Senior Care, a senior-focused care model serving older adults with complex and changing healthcare needs in the Greater Sacramento area. The model emphasizes panel ownership, longitudinal relationships, proactive outreach, care coordination, and meaningful contact among care teams, patients, and caregivers. Its goal is to support quality of life and reduce avoidable emergency department visits and hospital utilization when those interventions do not align with patients’ goals of care. Physicians will help shape a growing service line and practice in a team-based setting designed around older adults’ medical and social needs. The outpatient practice operates Monday through Friday, with approximately 10 pre-scheduled appointment slots per day using 30- and 60-minute visits, flexibility to add same-day appointments for urgent needs, and dedicated weekly administrative time. The care model includes same-day and next-day access and a shared 1:3 phone call expected to include evenings, weekends, and holidays. Physicians manage patient panels collaboratively and use clinical and care-team interactions beyond office visits to respond proactively as needs change. The interdisciplinary team is designed to include advanced practice clinicians, nurses, licensed clinical social workers, care coordinators, community health workers, clinical pharmacists, dietitians, physical and occupational therapists, and other clinical and community-based partners. Care focuses on prevention, chronic disease management, medication safety, care transitions, and maintaining quality of life. The position is suited to physicians who value relationship-centered primary care, long-term relationships with older adults and families, flexible and proactive patient management, and collaborative decision-making. Geriatrics training or experience is particularly well suited, though physicians with strong Internal Medicine or Family Medicine backgrounds and a genuine interest in older adults are also encouraged to apply. The position offers relocation assistance, a sign-on bonus, PSLF eligibility, benefits including zero-cost medical insurance, CME allowance, retirement and pension contributions, a customizable schedule, and access to EPIC.
