Physician, Geriatrics (Sutter Senior Care) (Sacramento, CA)

About The Position

Sutter Medical Group is seeking primary care physicians to join Sutter Senior Care, a senior-focused primary care model serving older adults with complex and changing healthcare needs throughout the Greater Sacramento area. The practice emphasizes panel ownership, long-term patient relationships, proactive outreach, care coordination, and meaningful engagement between care teams, patients, and caregivers. Its goals include helping patients maintain quality of life and reducing avoidable emergency department visits and hospital use when those interventions do not align with patients’ goals of care. Physicians will practice in a Monday–Friday outpatient setting with approximately 10 pre-scheduled appointment slots per day, using a mix of 30- and 60-minute visits. The model includes 60-minute new-patient and complex visits, 30-minute routine follow-ups, flexibility for urgent same-day visits, same-day and next-day access, and dedicated weekly administrative time. Physicians share a 1:3 phone call rotation, expected to include evenings, weekends, and holidays. The position offers an opportunity to help shape a growing senior-focused service line. Care is organized around shared accountability for a patient population rather than traditional visit-volume expectations. Physicians collaborate with an interdisciplinary team that may include advanced practice clinicians, nurses, licensed clinical social workers, care coordinators, community health workers, clinical pharmacists, dietitians, physical and occupational therapists, and other clinical and community partners. The care model addresses medical and social factors affecting older adults and supports coordinated care throughout the aging journey. The role is suited to physicians who value relationship-centered primary care, panel ownership, flexibility, proactive patient management, collaborative decisions, and care for older adults and their families. Geriatrics training or experience is particularly relevant; physicians with strong Internal Medicine or Family Medicine backgrounds and a genuine interest in older adults are also encouraged to apply.
