About The Position

The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community. Senior Care Partners PACE is looking for a compassionate and dedicated Physician Assistant to join our team. In this role, you’ll provide primary medical care to older adults in our program, working closely with a supervising physician and an interdisciplinary care team. Requirements

Graduate of an accredited Physician Assistant program.
Licensed as a PA in Michigan.
Bachelor’s degree required
At least 1 year of experience working with a frail or elderly population.
CPR certification (or willingness to obtain through our program).
Strong communication skills and the ability to work well within a team.
Compassion, patience, and a desire to improve the lives of older adults.
Comfortable using computers and electronic health record systems.
Reliable transportation; valid Michigan driver’s license preferred.

Nice To Haves

Master’s degree preferred.

Responsibilities

Assess and treat participants with episodic or urgent medical needs.
Document participant status and communicate updates to the physician and care team.
Work collaboratively with nurses, therapists, and other healthcare professionals to ensure coordinated, high-quality care.
Write medical orders and help manage ongoing care needs.
Support participants and their families using a patient- and family-centered approach.
Assist with general clinical duties as needed.
