Physician Assistant - Primary Care FMP
About The Position
We are seeking a compassionate and experienced Family Medicine Physician Assistant to join our dynamic team at Foundation Medical Partners / Southern NH Health. In this role, you will play a vital role in providing comprehensive care to a diverse patient population in Milford, NH. This is a W2, employed position with Foundation Medical Partners, a Not-for-Profit, 501(c) Organization. The role involves 40 hours per week, with 50% direct patient care and 50% coordinating and communicating patient care. It is a collegial practice with solid APP support and is a Clinical Affiliate of Massachusetts General Hospital. The position offers a hybrid work schedule with potential remote options once fully trained, and is conveniently located on the Southern NH/MA Border, minutes to the mountains or ocean.
Requirements
- Certified Physician Assistant
- 2+ years of Family Medicine experience
- Strong clinical skills, including assessment, diagnosis, and treatment planning.
- Excellent communication, collaboration, and interpersonal skills.
- Proficiency with EPIC EMR preferred
- Ability to work independently and as part of a team.
