About The Position

This position is for a Physical Therapist working day shifts from 8:00 AM to 4:30 PM in a Skilled Nursing Facility (SNF) environment with a guaranteed 35 hours per week. The Physical Therapist will provide rehabilitation services to residents recovering from injury, illness, surgery, or age-related functional decline. The role focuses on helping patients restore mobility, strength, balance, and overall independence in daily activities. The therapist will manage a caseload of residents requiring individualized therapy plans and collaborate with interdisciplinary healthcare teams to support rehabilitation goals. The position requires strong patient interaction, clinical evaluation skills, and the ability to adapt treatment strategies to meet the needs of residents in a long-term or post-acute care setting.

Requirements

  • Active Physical Therapist license in the appropriate state.
  • Graduation from an accredited Physical Therapy program with a Doctor of Physical Therapy or equivalent degree.
  • Strong knowledge of rehabilitation techniques including gait training, therapeutic exercise, mobility restoration, and fall prevention strategies.
  • Strong communication skills to coordinate care with nursing staff, physicians, and rehabilitation team members.
  • Strong documentation skills.
  • Familiarity with electronic medical record systems.
