About The Position

FOX Rehabilitation is hiring licensed Physical Therapy Assistants to provide hands-on care to older adults in Naples, Florida, and surrounding communities. Working under a supervising Physical Therapist, the PTA delivers individualized treatment plans through FOX’s Geriatric House Calls™ model in patients’ homes and senior living communities. The role focuses on therapeutic exercises and functional activities that help patients rebuild strength, mobility, confidence, and independence.

The PTA works as part of an interdisciplinary team alongside Physical Therapists, Occupational Therapists, Speech-Language Pathologists, and Fitness Specialists. The position offers mentorship from a supervising PT, collaborative care planning, and opportunities to develop clinical skills, grow a caseload, and advance a clinical career. Requirements

• Hold a valid Physical Therapist Assistant license in the state(s) of practice, or be eligible to obtain one.
• Graduate from an accredited Physical Therapist Assistant program.
• Have current CPR certification.
• Have basic computer skills and proficiency in Microsoft Office.
• Meet the physical demands of in-home and community-based care, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly; performing CPR; providing moderate to maximum assistance with patient transfers and bed mobility, including repositioning a bed-bound patient alone; standing, sitting, bending, kneeling, crouching, climbing stairs, and extensive walking; repetitive upper-extremity movements; driving to patient locations; and wearing PPE when required.

Responsibilities

• Deliver individualized treatment plans established by the supervising Physical Therapist, adjusting activities based on patient response and progress.
• Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities, while observing and tracking progress.
• Document visits accurately and on time in FOX’s EMR system and in accordance with FOX’s Clinical Documentation standards.
• Stay connected with patients, POAs, and community referral sources, and help coordinate coverage when needed.
• Provide in-home and community-based care, including visits to patients’ homes and senior living communities.
