About The Position
Activa Home Health is seeking a Field Physical Therapist Assistant to provide skilled physical therapy services in patients’ homes under the supervision of a Physical Therapist. This flexible per diem role offers an opportunity for permanent placement. The position supports patients in regaining independence and improving their quality of life through individualized treatment and coordinated care.
Requirements
- Active Florida PTA license.
- Degree from an accredited PTA program.
- Current CPR certification.
- Valid driver’s license and reliable transportation.
- Strong clinical and communication skills.
- Negative TB test or negative QuantiFERON blood test.
- Complete screening through the Care Provider Background Screening Clearinghouse.
Nice To Haves
- Home health experience preferred.
- Medicare experience preferred.
Responsibilities
- Provide skilled physical therapy services in patients’ homes under the supervision of a Physical Therapist.
- Implement individualized treatment plans to improve mobility, strength, balance, and function.
- Assist patients with therapeutic exercises and functional mobility training.
- Educate patients and caregivers on safe movement and home exercise programs.
- Monitor and report patient progress.
- Document visits accurately and on time.
- Collaborate with the interdisciplinary team to ensure coordinated care.
