About The Position

We are partnering with a well-established home health agency serving the King County, Washington area to find a skilled and dedicated Licensed Physical Therapy Assistant (LPTA) to join their home health team. This is a permanent, full-time opportunity for a clinician who values autonomy, meaningful patient interactions, and the opportunity to make a genuine difference in patients' lives in the comfort of their own homes. As a Licensed Physical Therapy Assistant, you will administer physical therapy to patients on an intermittent basis in their place of residence, in accordance with physician orders and the established plan of care, under the direct supervision of the Registered Physical Therapist. You will be an integral part of a collaborative home health team, working to help patients regain mobility, reduce pain, and achieve greater independence at home.

Requirements

  • Graduate of an accredited Physical Therapist Assistant program — required
  • Current Washington State PTA license in good standing — required
  • Home health or field-based clinical experience — a strong plus
  • Strong interpersonal, communication, and organizational skills
  • Ability to work independently and as part of a collaborative care team
  • Valid driver's license and reliable transportation — required

Responsibilities

  • Administer physical therapy treatments as directed by the supervising Registered Physical Therapist and in accordance with the physician-approved plan of care
  • Assist patients with therapeutic exercises, functional mobility training, and other skilled PT interventions in the home setting
  • Monitor patient responses to treatment and communicate progress and changes to the supervising PT
  • Prepare and submit accurate clinical and progress notes in a timely manner
  • Instruct patients and family members/caregivers on home exercise programs and safe functional activities
  • Collaborate with the broader home care team in patient care planning and care conferences
  • Maintain compliance with Agency policies, procedures, and all applicable regulatory requirements
  • Participate in in-service programs and Quality Assessment and Performance Improvement activities as assigned
  • Travel to and from patient homes throughout the service area
