About The Position

Adoration Home Health is seeking a dedicated Home Health Physical Therapy Assistant to provide patient-centered care in Lafayette, Tennessee, with coverage in Lafayette and Westmoreland. The PTA provides physical therapy services planned, delegated, and supervised by a qualified Physical Therapist in accordance with each patient’s Plan of Treatment. The role includes regular communication with the Physical Therapist, supporting patient progress and function, and maintaining required treatment and billing documentation. The position is full-time and offers flexible scheduling options, independent work with team support, and one-to-one patient care.

Requirements

• Current unrestricted license as a Physical Therapist Assistant in the state where practicing.
• Current Basic Life Support or cardiopulmonary resuscitation certification, in accordance with state regulations.

Nice To Haves

• A minimum of one year of work experience as a Physical Therapist Assistant is preferred.
• Knowledge of rehabilitation techniques related to complex neurological injury.

Responsibilities

• Provide physical therapy services planned, delegated, and supervised by a qualified Physical Therapist in accordance with the patient’s Plan of Treatment.
• Confer regularly with the Physical Therapist regarding the patient’s treatment plan, progress toward goals, questions, and concerns.
• Communicate patient needs and progress to the Physical Therapist of record.
• Assist in securing special adaptive equipment to improve patient function and incorporate adaptive equipment into the rehabilitation program.
• Maintain and submit patient treatment and billing documentation in the formats and timelines required by the company, facility, funding source, and local, state, and federal regulations.
