About The Position

As a Home Health Physical Therapist Assistant at CenterWell Home Health, you will report to the Clinical Manager and help patients regain strength, mobility, and independence in their homes. You will deliver personalized care focused on functional improvement and quality of life. The role involves carrying out treatment plans developed by a Physical Therapist, documenting care, participating in education and coordinating with the care team. The position is patient-facing and requires TB screening. It also participates in Humana's driver safety program and requires a valid driver's license and personal vehicle liability insurance.

Requirements

Current state license to practice Physical Therapy.
Valid driver's license, auto insurance, and reliable transportation.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Comprehensive knowledge of physical, emotional, social, and biological changes in the geriatric population.
Ability to read, write, and speak fluent English.

Responsibilities

Administer the Physical Therapist-developed treatment plan to patients in their homes.
Provide non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, and heat, light, cold, water, and electrical modalities.
Document client records and submit clinical documentation to the office on time each week.
Attend educational meetings and coordinate department in-services.
Notify the Physical Therapist or Branch Director of equipment failures, supply shortages, or changes involving staff or clients.
Apply infection-control concepts and universal precautions when performing patient care activities.
