About The Position

AHN Healthcare@Home is hiring a Physical Therapist Assistant (PTA) to provide skilled home health care to patients in the Erie, Pennsylvania area. The PTA provides compassionate, patient-centered physical therapy services in patients’ homes under the direction of a licensed Physical Therapist. The position offers independent field work supported by a dedicated field education team and collaborative leadership. It is eligible for a $5,000 first-year success bonus, with additional bonus opportunities beyond the first year.

Requirements

• Active Physical Therapist Assistant (PTA) license in the state where the employee works.
• One year of experience as a Physical Therapist Assistant and the clinical expertise to deliver quality patient care.
• Valid driver’s license.
• Ability to spend approximately 20% of the workday driving to and from patient locations.
• Successful completion of a drug screen prior to employment.
• Must be legally authorized to work in the United States.

Responsibilities

• Provide physical therapy services in the home health setting under the direction of a licensed Physical Therapist.
• Help patients restore mobility, reduce pain, and maximize functional independence through therapeutic exercises, gait training, transfer training, muscle re-education, and prosthetic or orthotic support.
• Educate patients and caregivers on the safe use of mobility and assistive devices.
• Monitor patient progress and communicate patient responses and changes in condition to the care team.
• Use critical thinking and problem-solving in real time.
• Complete thorough, compliant EMR documentation.
• Travel to patient homes within the assigned territory.
