About The Position

Stern at Home Therapy is seeking a Full-Time Physical Therapist Assistant (PTA) to provide maternity leave coverage at an established Assisted Living Facility (ALF) in Le Roy, NY. This role begins in October 2026 and is ideal for a PTA looking for a full-time, facility-based assignment with an established caseload. The PTA will work under the supervision of a licensed Physical Therapist to ensure continuity of care for patients within the facility.

Requirements

Active Physical Therapist Assistant license in New York required.
Graduate of an accredited Physical Therapist Assistant program.
Ability to commit to full-time coverage during the maternity leave period.
Strong clinical, communication, documentation, and time-management skills.
Ability to work effectively under the supervision of a Physical Therapist.
Ability to collaborate with facility staff and the interdisciplinary care team.

Nice To Haves

Geriatric, assisted living, or facility-based therapy experience preferred but not required.

Responsibilities

Provide one-on-one physical therapy treatments.
Carry out individualized plans of care established by the supervising Physical Therapist.
Help patients improve strength, balance, mobility, endurance, and functional independence.
Provide therapeutic exercise, gait training, transfer training, and fall-prevention interventions.
Monitor patient progress and communicate changes to the supervising Physical Therapist.
Educate patients and caregivers on exercises, mobility strategies, and safety.
Complete accurate and timely clinical documentation.
Communicate and collaborate with the supervising PT, facility staff, caregivers, and interdisciplinary care team.
Maintain continuity of treatment throughout the maternity leave coverage period.
