About The Position
South Creek Post Acute is seeking a passionate Physical Therapist Assistant (PTA) to join their supportive and energetic team. The PTA will be responsible for helping residents restore mobility, confidence, and independence. Under the direction of a licensed Physical Therapist, the PTA will provide hands-on care, implement treatment plans, and assist residents in achieving their therapy goals within a positive, team-oriented setting.
Requirements
- Licensed Physical Therapist Assistant in Washington State
- Strong communication and teamwork skills
Nice To Haves
- Experience in skilled nursing or post-acute rehab preferred
- Familiarity with Casamba or PointClickCare a plus
Responsibilities
- Deliver high-quality therapy services per the Physical Therapist’s care plan
- Document daily treatments and weekly progress notes accurately and on time
- Support restorative nursing training and participate in team meetings
- Educate residents, families, and caregivers in safe mobility and exercise routines
- Maintain treatment areas and equipment in good working order
- Communicate effectively with the care team regarding progress and needs
