About The Position

FeldCare Connects is seeking a self-motivated Physical Therapy Assistant to provide home healthcare to patients of all ages in the Los Altos area. This is an independent contractor position within FeldCare Connects’ app-based network, which connects partner agencies with therapists and uses software to support efficient patient care.

The Physical Therapy Assistant addresses patients’ needs related to mobility, pain reduction, restoring function, and preventing disability. Following the assessment and evaluation performed by a Physical Therapist, the assistant carries out the treatment plan, provides interventions to improve patient well-being, and evaluates progress. Services are provided in accordance with state laws, the plan of care, and coordination with the broader healthcare team.

Requirements

• Ability and enthusiasm to work with patients of all ages.
• Physical Therapy Assistant license and registration by the state.
• Completion of a Physical Therapy Assistant curriculum approved by the American Physical Therapy Association, the Council on Medical Education and Hospitals of the American Medical Association, or the Council on Medical Education of the American Medical Association.

Responsibilities

• Provide services within the scope of state laws governing Physical Therapy Assistants, in accordance with the plan of care and in coordination with other healthcare team members.
• Assist patients with therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report patient responses to treatment and changes in condition.
• Instruct patients, families, caregivers, and healthcare team members in therapy activities they can use to assist the patient.
• Implement the treatment plan based on the Physical Therapist’s assessment and evaluation, and evaluate patient progress.
