About The Position

Concordia Visiting Nurses is seeking a compassionate, motivated, and dedicated Physical Therapy Assistant to join its home health team. The PTA works alongside Physical Therapists to help patients regain strength, improve mobility, and remain safely independent in their homes. The role provides one-on-one care in patients’ homes, with flexibility to manage a daily schedule, and involves collaborating with an interdisciplinary healthcare team. Concordia Visiting Nurses is a Medicare-certified, state-licensed, Medicaid-approved home health agency serving nine Western Pennsylvania counties.

Requirements

• Graduate of an accredited Physical Therapist Assistant program.
• Hold a current Pennsylvania Physical Therapist Assistant license, or be able to obtain one before starting.
• Hold current CPR certification, or be able to obtain it before employment.
• Compassionate communication and the ability to build trusting relationships with patients and teammates.
• Strong clinical observation, documentation, and organizational skills.
• Ability to work independently while participating in an interdisciplinary healthcare team.

Responsibilities

• Provide physical therapy treatments under the direction of a licensed Physical Therapist and in accordance with the established plan of care.
• Help patients improve strength, balance, mobility, endurance, and functional independence.
• Implement therapeutic exercises, gait training, transfer training, and other evidence-based interventions.
• Educate patients and caregivers about exercise programs, fall prevention, safe mobility techniques, and proper use of assistive devices.
• Observe and document patient progress, and report changes to the supervising Physical Therapist.
• Collaborate with nurses, occupational therapists, speech therapists, physicians, and other healthcare professionals to provide patient care.
