About The Position

Concordia Visiting Nurses is seeking a compassionate, motivated, and dedicated Physical Therapy Assistant to join its home health team. The PTA works alongside Physical Therapists to help patients regain strength, improve mobility, and remain safely independent in their homes. The role provides one-on-one care in patients’ homes and offers autonomy in managing a daily schedule. Requirements

• Graduate of an accredited Physical Therapist Assistant program.
• Current Pennsylvania Physical Therapist Assistant license, or ability to obtain one before starting.
• Current CPR certification, or ability to obtain it before employment.
• Compassionate communication and ability to build trusting relationships with patients and teammates.
• Strong clinical observation, documentation, and organizational skills.
• Ability to work independently and as an active member of an interdisciplinary healthcare team.

Responsibilities

• Provide physical therapy treatments under the direction of a licensed Physical Therapist and in accordance with the established plan of care.
• Assist patients in improving strength, balance, mobility, endurance, and functional independence.
• Implement therapeutic exercises, gait training, transfer training, and other evidence-based interventions.
• Educate patients and caregivers on exercise programs, fall prevention, safe mobility techniques, and proper use of assistive devices.
• Observe, document, and report patient progress, and communicate changes to the supervising Physical Therapist.
• Collaborate with nurses, occupational therapists, speech therapists, physicians, and other healthcare professionals to provide patient care.
