About The Position

Avante at Melbourne Skilled Nursing and Rehabilitation Center is seeking a PRN Physical Therapist for its in-house therapy department. The therapist provides and directs Physical Therapy services to residents, including assessment, treatment, program planning and implementation, documentation, and communication. The role operates under physicians’ orders and includes coordinating resident assessments to support compliance with federal, state, and local regulations.

Requirements

Bachelor of Science in Physical Therapy from an accredited program.
Successful completion of the National Certification Examination for Registered Physical Therapist.
Current state licensure or eligibility for licensure.
0–3 years of long-term care experience.
Complete required background screening through the Agency for Health Care Administration (AHCA) Care Provider Background Screening Clearinghouse.

Responsibilities

Provide Physical Therapy services to residents and initiate referrals when appropriate and in accordance with department procedures.
Conduct resident screenings at regular intervals to determine the need for intervention or treatment.
Evaluate residents to gather information necessary for treatment planning and implementation.
Conduct specialized evaluations as indicated.
Maintain established confidentiality standards.
Implement and conduct treatment as outlined in the treatment plan.
Follow management direction.
Perform other duties as assigned.
