About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Per Diem Physical Therapist provides skilled physical therapy to patients in their homes to promote mobility, restore function, improve strength and balance, and enhance safety and independence. The therapist works collaboratively with patients, caregivers, and an interdisciplinary care team to support recovery, prevent hospitalizations, and improve quality of life in the home setting. The position offers compensation through per-visit-point pay for visits and an hourly rate for certain non-visit activities, such as orientation. The stated annual total cash target range assumes full-time employment at full productivity; actual compensation varies based on factors including local labor markets, education, work experience, productivity, and performance. Benefits and incentives are subject to eligibility requirements. Candidates must pass a drug test before beginning employment.
Requirements
- Graduate from a physical therapy educational program accredited by the Commission on Accreditation of Physical Therapy Education (CAPTE).
- Hold a current physical therapy license specific to the state assigned.
- Hold current CPR certification.
- Have at least 1 year of related physical therapy experience when entering home health care. Candidates with less than 1 year of experience require approval from regional clinical leaders and appropriate mentoring and/or participation in a residency program.
- Have a driver’s license and access to reliable transportation.
- Pass a drug test before beginning employment.
Responsibilities
- Help patients maintain, restore, and improve movement, activity, and function.
- Provide interventions, conduct reexaminations at least every 30 days, modify interventions as necessary, and perform discharge planning.
- Perform other duties as assigned.
