About The Position

Pinnacle Home Care is hiring a full-time Physical Therapist to provide home health services in the Vero, Vero Beach, and Beachside service area. The role focuses on compassionate, patient-centered care delivered in patients’ homes, helping them heal and regain independence through individualized therapy. The therapist works with physicians and an interdisciplinary care team, educates patients and caregivers, supervises Physical Therapy Assistants, and maintains timely, accurate clinical documentation. The position includes flexible scheduling and ongoing professional development.

Requirements

• Current state-specific Physical Therapist license.
• Graduate of an accredited physical therapy program.
• Valid BLS certification.
• At least one year of Physical Therapist experience.
• Excellent communication, organizational, and time-management skills.
• Valid driver’s license and access to a reliable, insured vehicle.
• Ability to visit homes that may not be wheelchair accessible.
• Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
• Ability to walk, stand, stoop, and occasionally lift or turn patients.

Nice To Haves

• Home health experience is preferred.
• Proficiency with electronic medical records.

Responsibilities

• Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
• Develop and implement individualized treatment plans in consultation with physicians and clinicians, adjusting them as needed.
• Collaborate with the interdisciplinary care team to optimize patient outcomes.
• Educate and support patients and families or caregivers on therapy programs.
• Maintain accurate clinical records and report changes in patients’ conditions to physicians.
• Supervise Physical Therapy Assistants in accordance with agency policies and state regulations.
• Participate in in-services, case conferences, and meetings to promote coordinated care.
• Identify patient and caregiver needs for additional services and make appropriate referrals.
• Confirm daily visit schedules to coordinate smoothly with other staff.
• Communicate patient progress and changes to physicians and care teams.
• Document visits and progress notes on the same day of service.
