About The Position

Pinnacle Home Care is hiring a full-time Physical Therapist to provide home health services in the Grand Island, Fruitland Park, Leesburg, and South Villages service areas. The role focuses on delivering individualized care in patients’ homes and supporting their recovery and independence. The therapist works with physicians, clinicians, and an interdisciplinary care team to develop treatment plans and coordinate patient care.

Requirements

• Current state-specific Physical Therapist license.
• Graduate of an accredited physical therapy program.
• Valid BLS certification.
• At least one year of Physical Therapist experience; home health experience is preferred.
• Excellent communication, organizational, and time-management skills.
• Valid driver’s license and access to a reliable, insured vehicle.
• Ability to visit homes that may not be wheelchair accessible.
• Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
• Ability to walk, stand, stoop, and occasionally lift or turn patients.

Nice To Haves

• Home health experience.
• Proficiency with electronic medical records.

Responsibilities

• Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
• Develop and implement tailored treatment plans in consultation with physicians and clinicians, adjusting plans as needed.
• Collaborate with the interdisciplinary care team to optimize patient outcomes.
• Educate and support patients and families or caregivers on therapy programs.
• Maintain accurate clinical records and report changes in patient condition to the physician.
• Supervise Physical Therapy Assistants in accordance with agency policies and state regulations.
• Participate in in-services, case conferences, and meetings to promote coordinated care.
• Identify patient and family or caregiver needs for additional services and make appropriate referrals.
• Confirm daily visit schedules to coordinate care with other staff.
• Communicate patient progress and changes with physicians and care teams.
• Document visits and progress notes on the same day of service.
