About The Position
Pinnacle Home Care is hiring a full-time Physical Therapist to provide home health services in the Vero, Vero Beach, and Beachside service area. The role focuses on delivering individualized, patient-centered care in patients’ homes, helping them heal and regain independence. The therapist works with physicians and an interdisciplinary care team to coordinate treatment and support positive patient outcomes.
Requirements
- Current state-specific Physical Therapist license.
- Graduation from an accredited physical therapy program.
- Valid BLS certification.
- At least one year of Physical Therapist experience.
- Excellent communication, organizational, and time management skills.
- Valid driver’s license and access to a reliable, insured vehicle.
- Ability to visit homes that may not be wheelchair accessible.
- Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
- Ability to walk, stand, stoop, and occasionally lift or turn patients.

- Proficiency with electronic medical records.
Responsibilities
- Perform initial and ongoing patient assessments, including OASIS evaluations and documentation.
- Develop and implement individualized treatment plans in consultation with physicians and clinicians, adjusting plans as needed.
- Collaborate with the interdisciplinary care team to optimize patient outcomes.
- Educate and support patients and families or caregivers about therapy programs.
- Maintain accurate clinical records and report changes in patients’ conditions to physicians.
- Supervise Physical Therapy Assistants in accordance with agency policies and state regulations.
- Participate in in-services, case conferences, and meetings to promote coordinated care.
- Identify patient and family or caregiver needs for additional services and make appropriate referrals.
- Confirm daily visit schedules to coordinate seamlessly with other staff.
- Communicate patient progress and changes with physicians and care teams.
- Document visits and progress notes on the same day of service.
